Indiana Contractor of the Year
Nominations may be submitted to the Executive Director or Awards Committee Chairperson. Applications must be received by January 5th.

	I would like to nominate ______________________________ for Indiana Contractor of the Year.

	The following questions should be answered in complete sentences on a separate sheet of paper.

	l. Contractor Description:
		A. How long has the above person been in business?
		B. Size of business in terms of employees and equipment?
	2. Status in Community
		A. How well is he/she regarded in the community?
		B. Participation in civic affairs (local chamber of commerce, PTA, community and service organizations)
		C. Has he/she been mentioned or featured or otherwise cited in local newspapers or in national or regional trade magazines for his/her accomplishments as a contractor?
	3. Contributions to Indiana LICA Chapter
		A. How long a member?
		B. History of attendance at state convention workshops
		C. Committees served on, Offices held in Indiana LICA
		D. Participation in LICA functions?

Nominations may be submitted to the Executive Director or Awards Committee Chairperson. Applications must be received by January 5th.

	I would like to nominate ______________________________ for Indiana Contractor’s Wife of the Year.

The following questions should be answered in complete sentences on a separate sheet of paper.

	l. What is her understanding of conservation contracting?
	2. What has she done to promote conservation in her area?
	3. How does she contribute to her husband’s work as a contractor, us INLICA member? (Even if she doesn’t work in the business, how does she support his efforts?)
	4. What community or civic projects or activities make her outstanding?
	5. Has she been active as a regular LICA supporter in the Indiana Chapter? (Did she assume any duties within the Indiana Chapter?)
